The many obstacles to this happening includes doctors and the current medical system, family members, and the patient. Properly managed through rich communication, these obstacles can be converted to resources for a good end of life. Selected forms. available on the internet, are reviewed and presented for consideration as well as one set redesigned and updated by the author. Two chapters provide essential and detailed information regarding the implications of available medical choices. Sometimes these choices may extend your physical life, but at very low quality. Repeatedly, you will be exposed to the issue of how complicated and stressful the decisions are and the emphasis will be that without the completion of Advanced Medical Directive Forms and communicating your wishes you are at the whim of those trying to help you. Another central emphasis is on the essential tasks of reflecting on what you want and communicating in depth with your medical proxy (health care agent), your family, and your doctor. This will prevent those who care from having to guess at what to do. You will also be exposed to dementia which is playing a greater role in considerations because people are living longer thanks to medical care... but not always with a quality of life. Dementia carries huge financial and psychological costs and is addressed through relatively detailed forms which instruct as to the wishes of the reader should s/he become unable to make decisions.
